A team has to choose which countries a trial runs in. Three things pull on the decision. How fast each country enrolls patients. How cleanly its data will combine with the rest. How much the country matters for the eventual launch. The natural instinct is to turn these into one “country score,” rank the list, and pick from the top.

That instinct is the mistake. The three things are not the same kind of number, and no single score can hold all three without secretly deciding how much each one is worth. The moment you collapse them, you have made the real decision and hidden it inside an average.

The clinical why

Speed, data poolability, and launch value are three different currencies, and they do not convert into each other. A country that enrolls fast may treat patients differently enough to strain the analysis. A country that matters commercially may enroll slowly. A country that is scientifically clean may be redundant for the launch. There is no exchange rate that tells you how many units of enrollment speed are worth one unit of clean data. That exchange rate is not a fact to look up. It is a strategic judgment someone has to make and own.
